Monika Kastner is the Research Chair in Knowledge Translation and Implementation at North York General Hospital in Toronto and Associate Professor at the Institute of Health Policy, Management and Evaluation and the Department of Family and Community Medicine at the University of Toronto. Monika’s goals are to improve the health of priority populations and to advance the science and practice of Knowledge mobilization and Implementation science. Her work is focused on making a positive impact on the health of older adults living with social frailty and multiple chronic conditions, including diabetes, using innovative e-health technologies that optimize healthcare delivery, clinical decision-making, and improved patient outcomes.
